Insurance Coverage
I am an out-of-network, fee-for-service provider. If you have out-of-network benefits through your insurance, services may be covered in full or in part.

To find out your out-of-pocket cost please follow the following steps:

Find your insurance company's phone number on your insurance card.

Ask if your plan includes out-of-network benefits for mental health care.

Ask if you have an out-of-network deductible before coverage begins​

Ask for the reimbursement rate for the appropriate service code below.

90834 - Individual Session (45 min)

Subtract the reimbursement amount from the total cost of appropriate service fee listed above. This will be your out-of-pocket cost for each session after reimbursement.
I will provide you with the required additional documentation (superbill) to attach to your insurance reimbursement claim.
SLIDING SCALE PAYMENT
In an effort to make counseling affordable, a limited number of appointment slots are available on a sliding-scale fee.

The fee is determined based upon the service provided and the client’s individual financial situation (eg., annual income, support payments, significant debt, etc.).
PAYMENT POLICY
Payment is due at the end of the session. I accept payment via Zelle and Venmo.
CANCELATION POLICY
If you need to cancel or change your scheduled appointment, please provide at least 48 hours notice and I will do my best to accommodate your schedule.

If we are unable to reschedule, you will be responsible for the session fee.
